Incident Recording, Evidence & Staff Welfare

LESSON 9 OF 10

Make a Factual Record

A good record may become important later.

Record times, locations, the person’s words where significant, observable condition, actions taken, staff involved, medical/police involvement and any evidence preserved. Separate fact from assumption. Serious disclosures can also affect staff; use supervisor/debrief and welfare support where needed.
