Accidents, Near Misses, Environmental Incidents & Learning
A near miss is a warning without the injury.
Report accidents, dangerous occurrences, near misses, spills and environmental incidents promptly. The purpose is to support immediate response and prevent recurrence through corrective action.
Immediate response+
Protect life, summon medical or specialist assistance and make the area safe where it is within your competence.
Preserve information+
Record location, time, conditions, witnesses, equipment and relevant photos/CCTV through authorised channels.
Near misses+
If a barrier nearly falls, a vehicle nearly strikes a pedestrian or somebody nearly slips, report it even if nobody was hurt.
Environmental incidents+
Spills, leaks, uncontrolled waste or pollution risks should be contained only if safe and trained, then reported through the site environmental procedure.
Good reporting should lead to review, corrective action and changes to training, equipment or procedures where needed.
