Good Information Drives Good Decisions
Reports should allow somebody who was not present to understand what happened.
Use concise radio messages and factual written reports. Record times, exact locations, descriptions, what you personally observed, actions taken, witnesses and who was informed. Preserve relevant evidence and do not share incident material on social media.
